Squamous cell carcinoma is the 2nd most common form of skin cancer in the United States and represents 20% of all skin cancers. SCC is most frequently seen in the elderly age population, but incidence is increasing in young individuals. Cutaneous SCC’s are locally invasive and destructive to surrounding tissues, but also carry the low risk (2-5%) of metastasis to regional lymph nodes or beyond.

SCC in situ presented as an erthematous , flat lesion

Firm, ulcerated SCC with nearby actinic keratosis ( a precursor lesion to SCC)

The primary risk factors for development of SCC include cumulative sun exposure and increasing age.

Approximately 50% of patients with a history of SCC will develop a second skin cancer within 5 years.

 

 

 

CLINICAL PRESENTATION:

  • 55% of cutaneous SCC develops on the head or neck, but any cutaneous surface may be affected
  • SCC’s more commonly present on sun-exposed areas

PREVENTION IS KEY!

  • Sun protection with daily application of SPF 30 to face, ears, neck, and chest
  • Use of sun protective clothing and hats
  • Sun avoidance during peak UV times
  • Routine full skin exam (every 6 months if history of SCC)

TREATMENT OPTIONS:

  • Biopsy all suspicious lesions
  • Refer confirmed cases of SCC for thorough evaluation and treatment

 

SQUAMOUS CELL CARCINOMA IN SITU Well demarcated, scaly, slow growing patch or plaque

CUTANEOUS HORN   A precancerous, hyperkeratotic horn-like papule. May also represent low-grade SCC

WELL DIFFERENTIATED INVASIVE SCC
A firm, hyperkeratotic papule with ulceration.

POORLY DIFFERENTIATED INVASIVE SCC
A fleshy, soft, granulomatous papule without hyperkeratosis
